Home Health Services

Recovering from an illness or surgery is tough business. Presbyterian Homes believes that one effective way for people to recover from surgery or hospitalization, or an acute illness, is in the comfort and security of their own home. The familiar surroundings, and the sense of home, provide an important therapeutic value in a person's recovery.

A multidisciplinary team approach

Often after hospitalization, or a stay in health care, clients may require the support of our home health professionals. We provide an array of services designed to continue rehabilitation and strengthen independence as a client recovers at home. Services include skilled nursing; physical, occupational, and speech therapy assistance with personal care from home health aides; medical social services from social workers; and rehabilitation therapy.

Skilled nursing care

Home health nurses visit patients in the home to manage the plan of care. Skilled assessment, teaching and hands-on care including: dressing changes, wound care, blood draws for lab tests, catheter changes and teaching can all be part of the care provided.

Home health aide services

Our home health aides visit clients who need assistance with bathing, grooming, ambulation and other personal care needs.

Rehabilitation services

Many clients leave short-term rehabilitation programs, but still require ongoing therapy services to restore and improve strength, mobility, functional skills - and independence. Presbyterian Homes offers complete rehabilitation services - physical, occupational and speech therapies in the home. Home safety evaluations by qualified therapists provide peace of mind to clients returning home after a stay in a rehabilitation program.

Social services

Hospitalization and rehabilitation can put serious stress on the patient and the family. Our social workers address issues that arise throughout a patient's recovery. Social workers can:

  • Assess social and emotional factors related to a client's illness.
  • Provide counseling based upon the above assessment.
  • Coordinate community services.

  • Medicare-certified home health agency.

QUALIFICATIONS FOR HOME HEALTH SERVICES

  • Physician must decide that skilled care is needed and make a plan for care at home.
  • Must need at least one of the following: intermittent (not full-time) skilled nursing care or physical therapy or speech language pathology services.
  • Must be homebound. This means that leaving home is a taxing effort.
